Philosophy
As the Emergency and Critical Care Center for the Kyoto Otokuni medical area, we provide safety and trust to citizens 24 hours a day, 365 days a year.
Basic Policy
- As a general rule, our Emergency and Critical Care Center will accept all severe and critically ill patients from Kyoto Otokuni and surrounding areas, including those with multiple medical conditions, on a 24-hour basis.
- Our Emergency and Critical Care Center adheres to the policy of not refusing to accept patients transported by ambulance as a general rule.
- Our Emergency and Critical Care Center is a referral hospital for primary and secondary emergency medical facilities and, as a general rule, accepts severe patients referred from these medical facilities for intensive care on a 24-hour basis.
- Patients whose initial treatment has been completed at our hospital will be transferred to regional medical institutions and nursing facilities in cooperation with them as early as possible to secure beds for transfer.
- Our Emergency and Critical Care Center will spare no effort to improve the emergency medical system in the Kyoto City area.
- Our Emergency and Critical Care Center actively provides clinical education in emergency medicine to medical students, clinical residents, nursing students, and paramedics.

About Emergency Medical Care
To Patients Wishing to Receive Emergency Care
Our Emergency and Critical Care Center provides medical care
for patients with high urgency and severe conditions.
Therefore, we need to prioritize examining patients with
more severe conditions. For this reason, the order of
examination may change, and waiting times may become very
long. Also, depending on the situation, we may ask you to
seek medical attention at another hospital.
Emergency physicians will provide emergency medical care
under the on-call system for surgery, internal medicine,
pediatrics, and obstetrics and gynecology, so we cannot
perform the same procedures and tests as in the general
outpatient clinic on weekdays.
Attending Physician
You cannot specify a physician from a specialized department.
Prescription Medication
As this is an emergency prescription, it will generally be for one day's supply (or until the next outpatient visit if it includes holidays). Additional prescriptions should be obtained from a physician in the specialized department at a later date.
Medical Certificate
As this is emergency treatment and examination, we cannot issue one. Please visit a specialized department at a later date and request it.
Follow-up Visit
In emergency care, the focus is on examining
life-threatening areas and areas of pain. For this reason,
pain may occur in areas different from those examined
during the emergency visit at a later date, and fractures
may become apparent over time.
In such cases, please be sure to see the relevant
department during the general outpatient clinic hours at a
later date.

Specialized physicians are not stationed at the hospital
during nighttime and holidays, so the same medical care as
during weekday morning hours cannot be provided.
Kyoto City has an emergency medical system for specialized
departments in pediatrics, internal medicine/ophthalmology,
and otolaryngology during nighttime and holidays, so please
also utilize the information below.

Rakuwakai Otowa Hospital Designated as Emergency and Critical Care Center!
Rakuwakai Otowa Hospital was designated as an "Emergency and Critical Care Center" by Kyoto Prefecture on March 30, 2012, after meeting the standards of the Ministry of Health, Labour and Welfare.
An "Emergency and Critical Care Center" is a facility designated as a "tertiary emergency medical institution" that provides advanced emergency medical care on a "24-hour, 365-day system" for all emergency patients, including those with severe and critically ill conditions requiring multiple medical departments.
As a tertiary emergency medical institution that serves as a referral center for critically ill patients, we will continue to work together with the local community and surrounding medical institutions to enhance emergency medical care and strive to meet the needs of the community.
Rakuwakai Otowa Hospital "Emergency and Critical Care Center / Kyoto ER"
Following the designation of Rakuwakai Otowa Hospital as an "Emergency and Critical Care Center," the name of the Kyoto ER Emergency and Critical Care Center has been changed to "Emergency and Critical Care Center / Kyoto ER".
It serves as a base for emergency medical care not only in the Yamashina area of Kyoto City but also throughout the entire Kyoto area, including the Kyoto South Interchange, Oyamazaki Interchange, and the Keiji Bypass, as well as connecting to the Kosei Road, due to its location next to the Meishin Kyoto East Interchange.

Cardiovascular Emergency
For sudden heart attacks such as angina pectoris, acute
myocardial infarction, and acute heart failure, prompt
diagnosis and treatment by a specialist are essential.

quality medical care by leveraging the strengths of both
departments.
Pediatric Emergency
Many people have experienced being at a loss when a child suddenly develops a fever or swallows a foreign object.
Our hospital provides pediatric emergency care on holidays
and at night, which is in high demand, and accepts a large
number of patients not only from the local area but also
from outside Kyoto Prefecture.
Please note that from April 2020, we will not be
providing pediatric emergency care on Mondays (8:00 PM to
8:30 AM the next day) and the first Sunday of the month
(8:00 PM to 8:00 AM the next day). We apologize for any
inconvenience.
Also handles acute stroke
For stroke, the faster and more accurate the diagnosis and the faster and more appropriate the treatment, the better the treatment results can be expected.
At our hospital, where CT and MRI necessary for diagnosis are available 24 hours a day, 365 days a year, we perform thrombolytic therapy (t-PA intravenous therapy) and aim for early detection and early treatment by responding immediately to hyperacute stroke.
Certified as a Primary Stroke Center
On September 1, 2019, our hospital was certified as a
Primary Stroke Center by the Japanese Stroke
Association.
The certification requirements include
the ability to perform t-PA intravenous therapy 24 hours a
day, 365 days a year for cerebral infarction, the
availability of head CT or MRI examinations, and the
presence of full-time specialists and a stroke treatment
team. As a base for stroke treatment, we will continue to
save the lives of as many stroke patients in the region as
possible and contribute to extending healthy life
expectancy.
Japanese Association for Acute Medicine Specialist Training Facility
Our Emergency and Critical Care Center / Kyoto ER is recognized for having advanced skills and systems that can train emergency specialists and is certified as a Japanese Association for Acute Medicine Emergency Physician Training Facility.
One of the conditions for becoming a specialist of the Japanese Association for Acute Medicine is to undergo clinical training for at least 3 years at a designated specialist training facility.
Acquired Emergency Medical Function
Our hospital was certified as having "Emergency Medical Function" by the Japan Council for Quality Health Care in January 2007, as a hospital equipped with staff, medical care, and facilities suitable for a highly advanced emergency medical institution.
Wide Area Disaster Support
Our hospital is registered as a "Wide Area Disaster
Emergency Medical Backup Center." In the event of a
disaster, we are linked with prefectural systems
nationwide for wide area disaster medical information such
as the number of patients that can be accepted and the
status of medical staff requests, which are necessary for
prompt and accurate medical relief activities. We have
also established a support system for wide area disasters
in addition to regional disasters.
We have introduced
a large-scale disaster rescue ambulance and also handle
the transportation of relief supplies and base life-saving
activities.
Triage Nurse
At Rakuwakai Otowa Hospital Emergency and Critical Care
Center / Kyoto ER, we have a "Triage Nurse" who interviews
and observes patients who arrive by means other than
ambulance to predict their condition and determine the
priority of examination based on urgency.
Triage means "priority allocation." It is a system for
providing more appropriate medical care according to
urgency.
ICLS Workshop
ACLS is advanced cardiovascular life support that utilizes advanced cardiopulmonary resuscitation methods such as airway intubation and drug administration, in addition to basic life support such as artificial respiration, cardiac massage, and defibrillation. At our hospital, we regularly hold ICLS workshops to help our staff contribute to regional emergency medical care, and we learn life-saving procedures together with doctors, nurses, and paramedics from regional medical institutions.
